NU Main Admissions Requirements

Freshmen

  • Complete Grade 12 Report Card (Form 138)
  • Certificate of Good Moral Character
  • Original copy of PSA-Authenticated Birth Certificate
  • Recent 2×2 Colored ID Picture with white background

 

For Philippine Educational Placement Test (PEPT) or Alternative Learning System (ALS) passers, additional requirements are as follows: Alternative Learning System (ALS)

  • Original Copy Certificate of Passing
  • Certificate of Rating
  • Certificate of Passing
  • Photocopy of Diploma
  • Original Admission Credentials from the last school attended prior to taking ALS (Original Report Card/Form 137)

 

For graduates of 2015 and below:

  • Complete Grade 12 Report Card (Form 138)
  • Certificate of Good Moral Character
  • Photocopy of the PSA Birth Certificate
  • Recent two (2) pieces 2×2 Colored ID Pictures

 

Transferee

  • Transcript of Records (TOR)/Copy of Grades

Second Degree

Graduate Studies

 

For Non-NU Graduates

  • Letter of Recommendation from Immediate Supervisor/HR Manager
  • Updated Copy of Resume
  • Original copy of PSA-Authenticated Birth Certificate

For NU Graduates

  • Photocopy of Official Transcript of Records
